Substitute Notice of Misdirected Mailings- December 2015

In December 2015,  Multispecialty Physician Partners’ (MPP)  accounts receivable department sent letters to approximately 497 patients who received services at one of MPP’s clinics in 2015 regarding its Advantage Patient Program – – of the 497 letters that were sent, 372 letters contained another patient’s first and last name.  No other protected health information was included in the letters.  Following the discovery of this error, MPP sent letters to all affected patients; however, MPP has discovered that not all affected patients received notice of this error.  To learn more information about this incident please click here.
